Dissemination of Class A Cephalosporinases and Class D Carbapenemases in Escherichia coli Isolates From a Tertiary Hospital in Sudan

Introduction The high prevalence of urinary tract infections (UTIs) and rising resistance to beta-lactam antibiotics, which is a global therapeutic concern, are caused by Escherichia coli (E. coli) extended-spectrum beta-lactamases (ESBLs) producers. It unclear how E. that produces ESBLs spreads throughout Gezira state, Sudan. study aimed evaluate the dissemination class A D genes among recognize antibacterial activity locally used cephalosporins carbapenems. Methods One hundred fifteen isolates uropathogenic were collected from patients who attended tertiary hospital. identified using colony morphology, gram staining, biochemical tests checked for 16S rRNA PCR. multidrug-resistant (MDR) testing was conducted agar disk diffusion. Finally, analyzed multiplex Results enrolled 200 with UTIs. found in 115 (57.5%) urine specimens examined, 60 (52.2%) them produced most antibiotics. antibiotic pattern higher against cefepime (100%), ceftizoxime (90%), cefuroxime (81.7%), ceftriaxone (81.7%) had lower meropenem (13.3%). genotypic characterization cephalosporinases 85% blaCTX-M, 70% blaSHV, 33.3% blaTEM, while carbapenemases, it 10% both blaOXA-23 blaOXA-51. Conclusion considerable increased predominance blaCTX-M blaSHV serious concerns health authorities. Meropenem could still be as drug choice ESBL-producing coli.
